New Module for Disability Applicants and New SSA-1560 Form Added

I've added a new module just for Social Security disability applicants, with multiple sections, providing a nice overview of the entire Social Security disability application process. I also added another form to the "SSA Disability Forms" section -  the ssa-1560 form for fee petitions, with fill-in-the-blank capability.
